Urgent care starts
Need home care this week?
Hospital discharge, a sudden change, a caregiver who can’t continue — when timing matters, call us first. We prioritize urgent starts, a nurse leads The Alberta Maple Care Assessment, and we’ll tell you honestly on the phone what’s possible and how fast.
Can’t call right now? Message us on WhatsApp or use the form below and mark it urgent — we treat these first.

Leaving the hospital?
Before discharge
Call us while your loved one is still admitted. We coordinate with the discharge plan, prepare the home, and can be in place for the first night back.
The first weeks home
Recovery support, medication reminders, mobility help, and a nurse watching for the complications that send people back to hospital. See Hospital-to-Home Recovery Support.
Respite, fast
If the family caregiver is suddenly unavailable (illness, travel, burnout), respite care can bridge the gap while we build a longer-term plan.

Tell us what’s happening
Three quick fields. Requests from this page are automatically flagged urgent — add the discharge date or timeline in the notes if you have one.
Urgent requests are handled first: a care coordinator responds within hours, not days. Your details are only used to contact you about care, never shared.
