Alberta Maple Care—a CDHCI (Type 3) provider registered with Alberta Blue Cross—to deliver your AHS-authorized care hours.

We submit claims directly to Alberta Blue Cross, so you don't have to worry about reimbursement.

How to Start CDHCI

Follow these four steps to begin managing your own care with Alberta Blue Cross.

Assessment & Consent

Meet with your AHS Case Manager to assess your needs, then sign and submit the Consent to Disclose Information form to Alberta Blue Cross.

Sign Agreement

Sign the CDHCI Letter of Agreement. You will then receive an approval letter from Alberta Blue Cross confirming your authorized hours and start date.

Choose Provider

Select a service provider agency from the registered Alberta Blue Cross vendor list that best fits your care requirements."

Manage Care

Direct your care according to your plan. You or your agency will submit claims directly to Alberta Blue Cross for reimbursement.

Who can be Hired?

When choosing a service provider, you must select an agency from the Alberta Blue Cross registered vendor list.

The contractual relationship is strictly between you (or your CDHCI manager) and your chosen agency. This model allows you to choose the delivery model that best meets your needs.
